1. Assumption of Risk

I understand that participation in weight training, bodybuilding, powerlifting, cardiovascular exercise, posing practice, stretching, personal training, group fitness activities, and the use of fitness equipment involves inherent risks, including but not limited to:

• Muscle strains and tears

• Sprains and ligament injuries

• Fractures and broken bones

• Joint injuries

• Heart attack or stroke

• Paralysis

• Permanent disability

• Death

I voluntarily choose to participate in these activities and knowingly assume all associated risks.

私は、ウエイトトレーニング、ボディビルディング、パワーリフティング、有酸素運動、ポージング練習、ストレッチ、パーソナルトレーニング、グループエクササイズ、およびジム設備の利用には、本質的な危険が伴うことを理解しています。

これらの危険には、筋肉や腱・靭帯の損傷、骨折、関節損傷、心臓発作、脳卒中、麻痺、後遺障害、死亡などが含まれますが、これらに限定されません。

I certify that I am physically able to participate in exercise or have consulted with a qualified healthcare provider regarding my participation.

I understand that Iron Shogun Gym does not provide medical advice, diagnosis, or medical evaluations.

I agree to stop exercising immediately if I experience chest pain, dizziness, difficulty breathing, severe discomfort, or any other concerning symptoms and seek appropriate medical attention.

私は、運動を行うのに適した健康状態であること、または必要に応じて医師等の専門家へ相談したうえで施設を利用することを確認します。

アイアンショーグンジムは、医学的診断、健康診断、医療上の助言を提供するものではないことを理解しています。

運動中に胸の痛み、めまい、呼吸困難、強い痛み、その他異常を感じた場合は、直ちに運動を中止し、必要な医療機関を受診します。

3. Release of Liability

3. 免責事項

To the fullest extent permitted by applicable law, I release and hold harmless Iron Shogun Gym, its owners, officers, employees, trainers, contractors, volunteers, and affiliates from liability for injuries, losses, or damages arising from the ordinary risks associated with participation in fitness activities and the use of the facility.

This agreement does not release Iron Shogun Gym from liability arising from gross negligence, willful misconduct, or any liability that cannot legally be waived under applicable law.

法令で認められる範囲内において、私は、通常の施設利用またはトレーニングに伴って生じる傷害、損害、損失について、アイアンショーグンジム、その経営者、役員、従業員、トレーナー、業務委託者、ボランティアおよび関係者に対して責任を追及しないことに同意します。

ただし、重大な過失、故意による不法行為、その他法律上免責が認められない事項については、この限りではありません。

4. Equipment Use

I agree to:

• Use all equipment only as intended.

• Follow posted rules and staff instructions.

• Return all equipment after use.

• Re-rack all weights.

• Wipe down equipment after use.

• Report damaged equipment immediately.

I understand that intentionally damaging equipment or using equipment in an unsafe manner may result in suspension or termination of my membership.

・器具は本来の用途に従って使用します。
・館内掲示およびスタッフの指示に従います。
・使用後は器具を元の場所へ戻します。
・プレートやダンベルは必ず元へ戻します。
・使用後は器具を清掃します。
・故障や破損を発見した場合は直ちにスタッフへ報告します。

故意による器具の破損や危険な使用方法は、会員資格停止または退会処分の対象となることを理解します。

5. Personal Property

I understand that Iron Shogun Gym is not responsible for the loss, theft, or damage of personal belongings brought onto the premises.

I am solely responsible for securing my personal property.

施設内へ持ち込んだ私物、貴重品、現金等の紛失、盗難、破損について、アイアンショーグンジムは責任を負わないことを理解します。

貴重品の管理は自己責任で行います。

I agree to:

• Treat members and staff respectfully.

• Maintain appropriate hygiene.

• Wear appropriate workout attire.

• Refrain from harassment, discrimination, violence, or threatening behavior.

• Follow all gym rules and staff instructions.

Violations may result in suspension or termination of membership.

・他の会員およびスタッフを尊重します。
・清潔な服装・衛生状態を保ちます。
・運動に適した服装で利用します。
・嫌がらせ、差別、暴力、威嚇行為を行いません。

7. Photography & Security

7. 撮影および防犯カメラ

I understand that security cameras are in operation throughout Iron Shogun Gym.

I agree not to intentionally photograph or record other members without their consent.

Commercial photography or filming requires prior approval.

館内には防犯および安全管理のため監視カメラが設置されていることを理解します。

他の会員の許可なく写真・動画を撮影しません。

商業目的の撮影や動画制作は事前にジムの許可が必要です。

8. Emergency Medical Authorization

If I become injured or unable to make medical decisions while at Iron Shogun Gym, I authorize staff to contact emergency medical services and my emergency contact.

I understand that I am responsible for any medical expenses incurred.

施設利用中に負傷または意思表示ができない状態となった場合、スタッフが救急機関および緊急連絡先へ連絡することに同意します。

その際に発生する医療費等は自己負担となることを理解します。

I agree to comply with all current and future Iron Shogun Gym membership policies, facility rules, and operating procedures.
